Articles of Organization of the undersigned, desiring to become a Non-Profit Private Foundation under the Non-Profit Law of New Jersey, do hereby certify:
The name of the Foundation shall be The Lindsay Wurtenberg Reflex Sympathetic Dystrophy Foundation, also to be known as The Lindsay Wurtenberg RSD Foundation.
The Place in this state where the principal office of the Foundation is to be in the Town of Mickleton, of Gloucester County.
The Lindsay Wurtenberg RSD Foundation is organized exclusively for charitable purposes, including, for such purposes,fundraising and the making of distributions to individuals who suffer fron RSD.
The purpose of The Lindsay Wurtenberg RSD Foundation is to perform fundraising activities by phone solicitation, e-mail solicitation, personal solicitation, by mail, and other activities, such as golf outings, and walk-a-thons, with the purpose of donating funds to individuals who suffer from Reflex Sympathetic Dystrophy, and will be receiving the ketamine-coma treatment. Awareness materials will always be provided at said fundraisers in order to educate the public about RSD.
Amendment: (As of March 19, 2006)
(a)Any person who suffers from RSD and is required to receive the ketamine-coma treatment, whether he or she be a family member of the Foundation, selection committee, officer, director, or contributor, is eligible for awards made under The Lindsay Wurtenberg RSD Foundation. In order to ensure an unbiased selection, the Grantee must be voted on and agreed upon by the five acting members of the Foundation. Severity of the disease, as determined by Doctor Robert Schwartzman, and financial necessity will be determining factors in choice of Grantee.
Amendment: (as of April 17, 2006)
Should the individual requiring the grant be related to a member of the Foundation, selection committee, officer, or director, that acting member may also be involved in the vote of determination. Each member of the Foundation eneterd into this agreement upon starting this Foundation and have again agreed upon this on April 17, 2006. Also, please see Conflict of Interest Policy.
Amendment: (As of April 26, 2006)
To ensure that funds raised are used specifically for the ketamine-coma treatment, the funds will be wired to the attention of Dr. Rohr at the Klinikum Saarbrucken in Saarbrucken, Germany, rather than given directly to the patient.
Amendment: (As of March 22, 2009)
Should a patient not be accepted in Germany for the ketamine-coma treatment, but has been chosen to be treated in Mexico for the ketamine-coma treatment, funds may be allocated for such reason. All committee members have agreed to this amendment. A wire transfer would then be forwarded to the attention of Doctor Cantu.

In order to be compliant with regulations for 501(c) (3) status,family members may not be involved in the vote of determination, as this would create a conflict of interest. Please see Conflict of Interest Policy.
(b)The five acting members of the Foundation will collect contributions as received from donations and fundraisers, to be deposited in an already established bank account at Wachovia Bank. It has been agreed upon that proceeds received are to be used for exempt purpose of aiding in medical costs for an RSD patient, as acceptable under section 501 (c) (3). Minimal funds will be used to acquire prizes to be given out at already organized Annual Golf Outing. Amount to be spent on prizes will be determined and agreed upon by the five members of the Foundation, after donations of prizes have been received by the Foundation.
(c) Records of all receipts and expenditures are being maintained, in order to show that said funds are being used for section 501 (c) (3) purposes.
RSD patients (from here to be known as grantees) being treated by Dr. Robert J. Schwartzman of Drexel Neurology in Philadelphia, will be candidates for funds (from here on to be called grants). Names of grantees will be submitted by Dr. Schwartzman to the Lindsay Wurtenberg RSD Foundation for consideration. Interviews with grantees will be conducted by the committee chairpersons. Degree of RSD severity and financial necessity will deternmine choice of grantee, which will be agreed upon by the committee and the chairpersons. Grant amount will depend on success of each fundraiser, and therefore, availability.
Upon dissolution of the Foundation, assets shall be distributed for payment of the ketamine-coma treatment for an RSD patient.
Amendment: (As of March19, 2006)
Upon dissolution of the Foundation, assets shall be distributed for one or more exempt purposes within the meaning of setion 501 (c) (3) of the Internal Revenue Code, or the corresponding section of any futurre federal tax code, or shall be distributed to the federal government, or to a state or local government, for a public purpose, such as chronic pain research.

It has been agreed upon by all committee members that the Foundation will begin a scholarship fund in the Foundation's name. This new award will go to a graduating senior/art student from Kingsway Regional High School, where Lindsay was a student. The award will be $1000.00, and can be used towards any expenses the student needs to further their art education. One award will be distributed this year, 2009.
